Note: To use the Internet within the Denver Public Schools network all students and their parents/guardians must sign a the Internet Use Agreement and Student Authorization Forms.

Field Trip Release and Permission Forms

1. Individual Teachers are responsible for handing out permission forms.
2. All Students must have a signed permission slip prior to leaving the building.
3. School uniforms are expected to be worn off campus during all field trips.
4. Students are responsible for their own lunch, unless otherwise notified.
5. Direct all questions to the Sponsoring Teacher(s)
